A Family Medical Leave is for when an employee has health issue of their own, or if the employee (you) are the primary caregiver of a spouse, child or parent with serious health conditions. In the meantime, here are a few items you should review first...
A few key items you should know:
- Your Request for Leave letter should be sent to Human Resource 60 days prior to the start of your leave of absence. Note: Most medical issues come as a surprise so make an appointment to meet with Human Resources to explain your need to take a leave of absence.*
- How many days do you have in your sick day bank? If you are unsure call the Payroll & Benefits Dept. at (732) 613-6758 or vial email at payroll@ebnet.org. Sick days are used prior to becoming eligible for a leave of absence, unless it is to tend to a family member. Sick days are applied only toward days that school is in session (do not include non school/workdays, i.e. winter/spring breaks, etc.).
- Physicians Letter - You will be required to submit a letter from your doctor stating the reason for requesting a leave of absence. If the leave is to tend to a family member then a letter explaining the condition as to why your need is necessary.*
- Disability Insurance – If you carry Prudential Disability Insurance, contact Prudential Rep. Cindy Cooper at (800) 727-3414 to notify her of your intended leave of absence and with any questions you may have regarding your plan.
